Pelviureteric junction (PUJ) obstruction/stenosis, also known as ureteropelvic junction (UPJ) obstruction/stenosis, can be one of the causes of obstructive uropathy. It can be congenital or acquired with a congenital pelviureteric junction obstruction being one of the commonest causes of antenatal hydronephrosis .

A point of care ultrasound was performed. The longitudinal view of the kidney shows mild hydronephrosis. A 6.8 mm stone is visible in the right PUJ. It is best seen in the longitudinal scan of the bladder, but visible on the transverse scan. With colour Doppler a small twinkle artefact is produced and the urinary jet is visible. Currently, most cases of PUJ obstruction are detected by pregnancy ultrasound while the baby is still in the womb.
Boys are affected with UPJ obstruction more commonly than are girls . Lesions are found more frequently on the left than on the right side. A 71-year-old woman was incidentally found to have hydronephrosis at ultrasound in 1992.

Patients with PUJ obstruction diagnosed by prenatal ultrasound should have a postnatal ultrasound. The timing of ultrasound is variable and depends on the severity of the PUJ obstruction. In general, however, ultrasound examination should be avoided in the first 2 days after birth, because hydronephrosis may not be detected because of extracellular fluid shifts that will underestimate the degree of hydronephrosis. Frequently, a PUJ obstruction will be detected incidentally on ultrasound or CT scans.
